As COVID-19 continues, so SWF Health & Social Care Group continues to work collaboratively with the Town’s GPs and Pharmacists. We also answer referrals from the national GoodSAM, Essex Wellbeing & Chelmsford Community Support Services. If things continue to worsen and restrictions increase with some sort of ‘circuit breaker’, local or national lockdown, we remain ready.
We’re providing health & social care support, information and signposting to patients & people in SWF and surrounds who are at risk, need to be cautious & stay at home, and are without a local support network.
South Woodham Ferrers Health & Social Care Group (SWFH&SCG) was established to look after the health and social care interests of the people of South Woodham Ferrers, recognising the roles of health and social care related professionals, volunteers and the Patient Participation Groups.
SWFH&SCG incorporates the SWFHC2000 Task Force, which convene when consultation or representation is required to consider major health and social care issues affecting the Town.
SWFH&SCG is open to anyone who lives, works, has a local need to access health and social care, or is involved in health and social care in SWF.
