The Essex Lifestyle, Community Agents, and local social prescribing services are offering support to anyone who wants to make lifestyle changes to improve their health and well-being, find local services, or need support with living independently

For example; by improving your diet, increasing your exercise and addressing low self esteem, you could significantly reduce any risk of cardiovascular disease and mental health illness.

The Care Navigation Partnership offers free and confidential support for people aged 16+ who live in Essex and may benefit from help with their health and well-being.

The team can help with:

  • Accessing community activities
  • Boosting confidence and personal resilience
  • Giving up smoking
  • Healthy lifestyle and diet
  • Housing, welfare benefits and accessing statutory services
  • Looking after someone
  • Mobility and equipment
  • Personal safety and security
  • Physical activity
  • Reducing social isolation
  • Self-management and self care
  • Volunteering opportunities

What to expect:

  • A telephone call to discuss potential services available
  • Coordination to find suitable support
  • Follow-up contact to ensure that you receive the right support
